It's a new year - and with a new year, comes the need for updated BSA Medical forms.
Troop 889 and the BSA Part A & B medical form be on file before attending ANY Troop activity. This is a requirement for both Scouts and adults attending events.
These forms are only good for 12 months, so they must be filed every year. Even if your medical forms from last year have, your insurance information may have changed as of January 1, so it is important that you turn in new forms as soon as possible.
Summer Camp - Medical Form Part C: This form, to be filled out by your Doctor, is required for all events longer than 72 hours. This is required for Summer Camp, and any high-adventure. These are also good for 1 year, but can be turned in independently of Parts A/B above, closer to summer camp.
High Adventure - Medical Form Part D: This form is required for any high-adventure activities in addition to the Parts A-C, and can be turned in independently as required.
Medical forms, and the BSA policy on medical forms can be found HERE.
The PDF's can be typed into directly, and printed as needed. These forms change often, so check for updates. For convienence, you can copy/update prior years forms that you have saved, as long as the BSA forms have not changed.
